TIMP-2/MMP-2平衡对胃癌浸润转移的影响和意义

摘要 目的 研究基质金属蛋白酶及其抑制剂 (MMP 2 /TIMP 2 )与胃癌TNM临床分期及血管生成 (angiogenesis)表达的相关性。方法 应用免疫组织化学S P法 ,分析 4 0例手术切除的新鲜胃癌标本MMP 2 /TIMP 2、CD34的表达。结果 TIMP 2在胃癌癌组织中阳性表达率为 35 % (14 /40 ) ,低于癌旁组织 5 7 7%的阳性率 (χ2 =4 .0 73,P <0 .0 5 ) ;MMP 2在肿瘤组织中表达率为 6 7.5 % (2 7/40 ) ,癌旁组织为 4 2 .5 % (17/40 ) ,两者比较差异显著 (χ2=5 .0 5 1,P <0 .0 5 )。MVD在胃癌组织中表达增加 (32 .5 6± 9.5 7)明显高于癌旁组织 (18.79± 8.4 1) ,二者比较差异均有显著性 (P <0 .0 1) ;TIMP 2 /MMP 2比值在肿瘤组织中 (0 .76± 0 .2 3)较癌旁组织 (1.5 3± 0 .32 )明显降低 (P <0 .0 1) ,与TNM分期密切相关 (P <0 .0 5 ) ,并与MVD呈明显负相关 (γs =- 0 .2 5 3,P <0 .0 5 )。结论 TIMP 2 /MMP 2在人体胃癌组织中的表达失衡 ,其可能通过干扰了细胞外基质的降解 ,进而影响肿瘤血管的生长、血管形成而在胃癌的发生和演进中起到重要作用。 Objective To investigate antiangiogenic effects of balance of TIMP-2/MMP-2 on gastric carcinoma.Methods 40 cases of gastric carcinoma patients were examined with immunohistochemical staining (SP-method)by using anti- MMP-2/TIMP-2 and anti-factor CD34(+) monoclonal antibodies.Results TIMP-2 showed a percentage of positive expression with 57.7% (23/40) in adjacent tissues of gastric cancer,which was higher than that in its primary foci 35%(14/40) of gastric cancer(P <0.05). MMP-2 levels were significantly increased in tumors compared to normal tissues (P <0.05). MVD was higher in tumors compared to normal tissue(P <0.01). The TIMP-2/MMP-2 ratio was higher (P < 0.01) in tumors compared to normal mucosa. TIMP-2/MMP-2 ratio was closely correlated with TNM staging of gastric cancer (P <0.05) and was also positive correlation with the mean MVD(P <0.05).Conclusion There must be a certain antagonist between the expression of MVD and the ratio of TIMP-2/MMP-2 in human colon cancer by interfering with the process of degradation of extracellular matrix and the angiogenesis,TIMP-2/MMP-2 play an important role in growth,invasion,metastasis and angiogenesis in colon cancer .
