心尖肥厚型心肌病的心电图表现

The characteristics of the electrocardiography in patients with Apical Hyperotrophic Cardiomyopathy

目的 :探讨心尖肥厚型心肌病 (AHCM )的心电图特点。方法 :对 16例心尖肥厚型心肌病患者 (男 14例 ,女 2例 )进行心电图、超声心动图及冠状动脉造影和左室造影等检查 ,并与正常人对照。结果 :心尖肥厚型心肌病心电图显示胸导联V3～V5R波振幅增高 ,胸导联ST段下移 ,以V2～V5最明显 ,T波对称性倒置具有TV4≥TV5 >TV3的特征 ,应进一步做超声心动图检查 ,多可明确诊断 ,必要时做冠状动脉造影或放射性核素心肌断层显像 ,以提高诊断率。结论 :标准 12导联心电图显示胸导联V3 V5R波振幅增高伴ST T改变的特征性改变 。

Objective:To study the characterics of ECG in patients with apical hypertrophic cardiomyopathy (AHCM).Methods:sixteen patients (male 14, female 2) with AHCM and 32 normal subjects were compared with ECG and echocardiography,some patients underwent coronary arteriography and left ventriculography.Results:The AHCM patients' ECG showed that the amplitude of R wave markedly increased in the V3～V5 leads,ST segment depressed significantly in the chest leads, specially in V2～V5 leads.The characteristic of the ECG in the AHCM patients was giant negative T wave and T wave inversion V4≥V5>V3.The diagnosis was confirmed if echocardiography was taken,sometimes the coronary arteriography or radionuclear myocardial imaging was necessary to confirm the diagnosis.Conclusion:The AHCM should be suspected if high R wave in the leads V3～V5 with ST segment depression and very deep T wave inversion.
