血清基质金属蛋白酶-9、白细胞计数与脑梗死体积及近期预后的关系

Prognostic significance of matrix metalloproteinases-9 and leukocyte count in acute cerebral infarction

目的 研究血清基质金属蛋白酶 9(MMP 9)水平、白细胞计数与缺血性脑卒中的梗死灶体积大小以及预后的关系 ,并探讨其潜在的机制。方法 对 12 4名缺血性脑卒中患者在发病后 2 4h抽取肘静脉血 ,用酶联免疫吸附测定法测定MMP 9水平 ,并进行白细胞计数。根据脑卒中患者临床神经功能缺损程度评分标准对患者入院以及出院时进行评分 ,所有患者均进行CT/MRI检查。结果 脑梗死组MMP 9水平及白细胞计数均明显高于腔隙性脑梗死组以及对照组。相关分析结果表明 ,MMP 9水平与预后呈等级正相关 (r =0 .180 ,P =0 .0 4 6 )。白细胞数增高组脑梗死与腔梗构成比、病死率、MMP 9水平明显高于白细胞数正常组 ,而治疗有效率则明显低于白细胞数正常组。结论 脑梗死急性期血清MMP 9水平。

Objective To investigate the relation among matrix metalloproteinases 9 (MMP 9), leukocyte, the infarction volume and prognosis.Methods 124 patients with ischemic cerebral stroke were included in our study.All patients were scored according to the criteria of the Clinical Neurological Function Deficient (CNFD) and identified by CT or MRI.Serum MMP 9 levels in vein blood samples after 24 hours of ischemic stroke were detected by enzyme linked immunosonbent assay and the number of leukocyte was counted.Results Serum MMP 9 levels and leukocyte count in the territory infarction group were significantly higher than those in the lacunar infarction group and control group ( P < 0.01 ).Correlation analysis showed that there was a positive correlation between MMP 9 and prognosis ( r = 0.180 , P = 0.046 ).Serum MMP 9 levels, mortality and the component ratio of infarction in high leukocyte count group were all higher than those in moral leukocyte count group while the curative effect was significantly lower.Conclusions There may be a close relation among MMP 9 levels, leukocyte count, the infarction volume and prognosis.
