乳腺癌、胃癌及大肠癌患者外周血CK20的检测与其临床病理分期相关性研究 被引量:6

摘要 目的 :检测乳腺癌、胃癌及大肠癌患者外周血中细胞角蛋白 2 0 (cytokeratin2 0 ,CK2 0 )并分析其临床病理联系。方法 :应用逆转录聚合酶链反应 (RT -PCR)检测乳腺癌、胃癌及大肠癌患者外周血CK2 0mRNA ,并以 6 0例健康者作为对照。结果 :6 0例健康对照组外周血CK2 0mRNA均为阴性。实验组 138例CK2 0mRNA表达阳性为 5 3/ 138,其中CK2 0mRNA表达阳性分别为乳腺癌 16 / 4 4 ,胃癌 2 2 / 5 7及大肠癌 15 / 37。CK2 0mRNA表达阳性在TNM及Dukes(大肠癌 )分期中分别为 :I期 (乳腺癌 1/ 6 ;胃癌 2 /11) ,II期 (乳腺癌 2 / 12 ;胃癌 4 / 19) ,III期 (乳腺癌 10 / 18;胃癌 12 / 2 1)及IV期 (乳腺癌 3/ 8;胃癌 4 / 6 ) ,(P <0 .0 5 ,I,IIvsIII,IV) ;大肠癌A期 (1/ 7) ,B期 (4 / 16 ) ,C期 (7/ 10 )及D期 (3/ 4 ) ,(P <0 .0 5 ,A、B及C、D期间 )。胃癌及大肠癌病理改变在低未分化及中高分化癌间经 χ2 检验有显著差异。结论 :RT -PCR方法检测乳腺癌、胃癌及大肠癌患者外周血中CK2 Objective: To explore the cytokeratin 20 (CK20) in peripheral blood of the patients with breast,gastric and colorectal cancer and evaluate the clinical significance. Methods: The expression of CK20mRNA in peripheral blood was examined by reverse transcription polymerase chain reaction (RT-PCR) in 138 patients with breast, gastric or colorectal cancer and 60 normal controls. Results: In peripheral blood of 60 healthy donors, no CK20mRNA was expressed. The positive CK20mRNA were detected in breast(16/44),gastric(22/57) and colorectal(15/37) cancers patients. The expression of CK20mRNA in TNM and Dukes(colorectal cancer) clinical staging were found in I (1/6 in breast and 2/11 in gastric cancer),II(2/12 in breast and4/19 in gastric cancer),III(10/18 in breast and 12/21 in gastric cancer) and IV(3/8 in breast and 4/6 in gastric cancer),(P<0.05,I,II vs III,IV) and in colorectal cancer A(1/7),B(4/16),C(7/10) and D(3/4),(P<0.05,A,B vs C,D). There were significant difference of the CK20 expressions rate between the patients with poor differentiated, high and middle differentiated cancers. Conclusion:The expression of CK20mRNA in peripheral blood of patients with breast,gastric and colorectal cancers is correlated with clinical staging and cellular differentiation. The detection of CK20mRNA by using RT-PCR are useful marker of circulating tumor cell and will be helpful in assessing prognosis, tailoring therapy, or developing new strategies.
