慢性阻塞性肺疾病合并肺间质纤维化的临床研究

Clinical analysis of chronic obstructive pulmonary disease with pulmonary interstitial fibrosis

目的 探讨慢性阻塞性肺疾病(COPD)合并肺间质纤维化(PIF)的特点及二者之间的关系。方法 分析三年来我院确诊的25例COPD合并PIF患者的病史、体征、X线胸片、高分辨CT(HRCT)、肺功能和血气分析结果。结果 COPD合并PIF患者多具有较长的吸烟史,临床表现介于上述两种病变之间,其肺功能测定多为混合性通气功能障碍,X线胸片、HRCT兼有两种疾病的特点。结论 尽管COPD和PIF是两种不同的疾病,但是可以并存,且具有其独特的临床表现,胸部HRCT能提供重要的临床诊断依据。

Objective To compare the clinical characteristics and analyse the relationship between the chronic obstructive pulmonary disease ( COPD) and pulmonary interstitial fibrosis( PIF). Methods Twenty-five patients with COPD and PIF were collected for study in the past three years. Clinical symptoms, radiographic features, arterial blood-gas analysis data, lung function tests and CT of the chest were analyzed. Results These patients had a long history of cigarette-smoking. Their clinical features were those of both COPD and PIF . The lung function tests showed mixed ventilation disturbances. X-rays and hight resoulotion computed tomography (HRCT) demonstrated some features of both COPD and PIF. Conclusion Although COPD and PIF are two different diseases, they could coexist in some patients. Their clinical features do not like either of the two diseases. HRCT is an important method for detecting the coexistance of COPD and PIF .
