摘要
目的 研究流式细胞仪 (FCM)检测肺癌穿刺标本p5 3蛋白、增殖细胞核抗原 (PCNA)表达的价值。资料与方法 运用FCM检测 6 6例穿刺标本p5 3蛋白、PCNA表达。结果 (1) 6 6例肺肿块中病理诊断恶性 5 3例 ,13例病理诊断及临床随防诊断为良性病变。 (2 ) 5 3例肺恶性病变p5 3蛋白表达阳性率为 6 2 .3% ,PCNA阳性率为4 5 .3% ;13例良性病变p5 3蛋白表达阳性率为 30 .8% ,PCNA阳性率为 7.7% ;两组p5 3蛋白、PCNA阳性表达有显著性差异 (P <0 .0 5 )。 (3) 5 3例肺恶性病变穿刺敏感性为 94 .3% ,13例肺良性病变穿刺特异性为 76 .9% ,6 6例穿刺诊断准确性为 90 .9% ,气胸发生率为 4 .6 %。结论 运用FCM可以检测肺癌穿刺标本p5 3蛋白、PCNA表达 ,为了解肺癌的分子生物学行为特性提供帮助。
Objective To evaluate flow cytometer (FCM) in estimating the expression of p53 protein and proliferating cell nuclear antigen (PCNA) in lung cancer specimen obtained by needle biopsy.Materials and Methods CT-guided percutaneous needle biopsy of lung lesions was performed in 66 patients with lung mass. The expression of p53 protein and PCNA was determined by FCM.Results (1) Of 66 cases, pathologic malignancy was confirmed in 53, and benign lesion in 13. (2) For 53 malignant cases, the positive expression rate of p53 and PCNA was 62.3% and 45.3%, respectively. For 13 benign lesions, the positive expression rate of p53 and PCNA was 30.8% and 7.7%, respectively. Significant difference in the expression of p53 and PCNA existed between malignant and benign lesions (P<0.05). (3) The sensitivity for malignant and benign lesions was 94.3% and 76.9%, respectively. The diagnostic accuracy for 66 cases was 90.9%. Pneumothorax occurred in 3 cases (4.6%). Conclusion By using FCM, the expression of p53 protein and PCNA in puncturing biopsy specimen can be estimated, which is helpful for understanding the molecular biologic behavior of lung cancer.
