认知行为干预治疗对乳腺癌患者免疫功能的影响

Effect of cognitive behavioral intervention therapy on immunological function of patients with breast cancer

目的:探讨改良行为干预疗法对乳腺癌术后辅助化疗患者的免疫功能是否存在积极的调节作用。方法:对来自广东湛江地区于广东医学院附属医院就诊的42例乳腺癌术后患者实验组,进行CMF方案辅助化疗及音乐治疗加放松内心意()象训练干预治疗,及时调整患者的不良心理状态,测定干预前后患者的免疫功能,并与未加认知行为干预的44例乳腺癌术后患者(对照组)做对照。结果:实验组治疗后CD4(32.38±9.11)和CD4/CD8值1.67±0.76)较(化疗前有明显提高,差异有显著性意义(t=3.06～5.66,P<0.05),对照组化疗后CD3(36.73±11.01),CD4(25.19±7.35),CD4/CD8(1.32±1.01)及NK细胞活性21.36±12.86)均较化疗前明显下降,差(异有显著性意义(P<0.01)。干预组化疗后CD3,CD4,CD4/CD8及NK细胞活性高于对照组,差异有显著性意义(t=-1.22～2.07,P<0.01)。结论:音乐治疗加放松内心意象训练的认知行为干预疗法对乳腺癌化疗患者的免疫功能调节存在积极的作用。

AIM:To explore whether the cognitive behavioral intervention therapy has the positive effect for adjustment on the immunological function in adjuvant chemotherapy patients with breast cancer after operation. METHODS:After operation,forty two patients with breast cancer(experimental group)(from clients of Zhanjiang in the Affiliated Hospital of Guangdong Medical College)received comparative mortalify figure(CMF)adjuvant chemotherapy and musicotherapy plus relaxation with guided imagery,and the bad mental status of the patient was adjusted.The immunological function was detected before and after intervention,which was compared with that of 44 non intervention patients with breast cancer(control group). RESULTS:CD4(32.38±9.11) and CD4/CD8(1.67±0.76) in the experimental group after chemotherapy were obviously higher than those before chemotherapy,and there was a significant difference (t=3.06 to 5.66,P< 0.05);CD3(36.73±11.01),CD4(25.19±7.35),CD4/CD8(1.32±1.01)and NK cell activity(21.36±12.86) in the control group after chemotherapy were obviously lower than those before chemotherapy,and there was a significant difference (P< 0.01).CD3,CD4,CD4/CD8 and NK cell activity in the intervention group after chemotherapy were higher than those of the control group,and there was a significant difference between the two groups(t=-1.22 to 2.07,P< 0.01). CONCLUSION:The cognitive behavioral intervention therapy of musicotherapy plus relaxation with guided imagery plays a positive role in immunological function of patients with breast cancer who receive chemotherapy.
