体外冲击波定位方法中超声定位与痛点定位的疗效比较

Comparison of the effects between ultrasonic orientation and pain point orientation among the orientations of extracorporeal shock wave therapy

目的:体外冲击波疗法(extracorporealshockwavetherapy,ESWT)治疗软组织疾病的定位方法,仍有争议,通过与痛点定位方法比较探讨超声定位在ESWT治疗肱二头肌长头腱炎中的应用价值。方法:本组肱二头肌长头腱炎86例均采用ESWT治疗,其中31例采用超声定位、55例采用痛点定位。比较应用两种定位ESWT治疗肱二头肌长头腱炎的疗效。结果:超声检查观察了肩部结构、肱二头肌长头腱炎的二维声像图特征。ESWT治疗后随访个月,超声定位6ESWT及痛点定位ESWT治疗肱二头肌长头腱炎疗效优良率分别为92.3%及79.1%,差异有显著性意义(χ=6.12,P<0.05)。2结论:ESWT超声定位可提高ESWT治疗肱二头肌长头腱炎的疗效,在冲击波治疗软组织疾病时,掌握和应用超声定位具有重要的意义。

AIM:Treating the soft tissue diseases with extracorporeal shock wave therapy (ESWT) is still disputed.The aim of the study was to probe into the value of ultrasonic orientation for treating the inflammation of long tendon of biceps muscle(ILBM)compared with pain point orientation. METHODS:Eight six cases of ILBM were treated with ESWT.Thirty one cases of ILBM were treated with ESWT of ultrasonic orientation,and other 55 cases treated with ESWT of pain point orientation. The effect was compared between ultrasonic orientation ESWT and pain point orientation ESWT. RESULTS:The characters of shoulder structure and ILBMwere observed through two dimensional ultrasonnogram by ultrasonic examination.After 6 month follow up, the results showed that the satisfactory rates of effect were respectively 92.3% and 79.1% with ultrasonic orientation ESWT and pain point orientation ESWT in the cases of ILBM,and the difference of satisfactory rates was significant between the ultrasonic orientation ESWT group and pain point orientation ESWT group(χ2=6.12,P< 0.05). CONCLUSION:The effect of ultrasonic orientation ESWT was better than that of pain point orientation ESWT for ILBM.The ultrasonic orientation is of important significance in grasping and using of ESWT for soft tissue diseases.
