持续静脉输注国产异长春花碱加顺铂治疗晚期非小细胞肺癌临床观察

Cisplatin and continuous infusion homemade vinorelbine combination chemotherapy for advanced Non-small cell lung cancer.

目的 :观察持续静脉输注国产异长春花碱 (盖诺 )加顺铂治疗晚期非小细胞肺癌的疗效及毒副反应。方法 :盖诺 10mg静脉推注第 1天同时盖诺 10mg通过深静脉置管采用静脉泵 2 4小时给药第 1～ 5天 ;顺铂 4 0mg静脉滴注第 1～ 3天。 2 1天为 1周期 ,至少连用 2周期评价疗效及毒副作用。结果 :全组 36例 ,无完全缓解 (CR) ,部分缓解 (PR) 19例 ,病情稳定 (SD) 14例 ,疾病进展 (PD) 3例。总有效率 (CR +PR) 5 2 8%。主要毒副反应为白细胞下降及消化道反应。Ⅲ～Ⅳ度白细胞下降为 9例 (2 5 % ) ,Ⅱ度白细胞下降 10例 (2 7 8% )。Ⅲ度消化道反应 2例 (5 5 % ) ,Ⅱ度消化道反应为 7例 (19 4 % )。神经毒性轻微。 1例出现Ⅱ度肝功能损害。结论 :持续静脉输注盖诺加顺铂治疗晚期非小细胞肺癌疗效较高 ,毒副反应较传统方法较小。

Objective:To evaluate the curative effect and toxicity of cisplatin plus continuous infusion homemade vinorelbine in the treatment of advanced non-small cell lung cancer.Methods:Candidate patients are advanced NSCLC. Vinorelbine is infused continuously via the cathetervenae subclavia.Regimen: NVB 10mg iv bolus day1, NVB 10mg iv continuous infusion 24h, day1-5; PDD 40mg iv, day 1-3. each cycle was 21 days and the clinical response was assessed after at least two cycles.Results:It showed that PR 19, SD 14, PD 3 cases with an overall effective rate of 52.8%. The main side effects included neutropenia: grade Ⅲ-Ⅳ 25%, nausea and vomiting 41.7%(15/36). GOT elevation 2.8%(1/36). Conclusion:Cisplatin plus continuous infusion homemade vinorelbine is effective for advanced NSCLC and toxicities are milder than the classical usage of vinorelbine.
