月经第3天瘦素水平与体外受精-胚胎移植结局的关系 被引量:3

摘要 目的 :了解基础瘦素 (leptin)水平是否对体外受精 胚胎移植 (IVF ET)结局具有预测价值。方法 :选择90名不孕妇女共 90个IVF周期 ,比较妊娠组与非妊娠组血清激素水平及临床资料的差异。按leptin/体重指数(BMI)值分为 3组 ,比较各组IVF ET周期临床指标的差异。结果 :妊娠组具有较低的基础leptin、leptin/BMI水平。低leptin/BMI组具有较多的周期获卵数 ,较高的受精率、胚胎种植率、临床妊娠率。当leptin/BMI >0 .65时妊娠率明显降低。结论 :基础leptin/BMI对IVF ET结局具有较好的预测性。高血leptin会影响卵子与胚胎质量 ,从而使着床率降低 ,妊娠率也随之降低。 Objective To explore the predictive va lue of the basal serum leptin levels to in vitro fertilization and embryo transf er (IVF-ET) outcome in infertile women. Methods Ninty infertile women who receive the IVF-ET treatme n t were chosen. To compared the serum hormone levels and clinical indices between the pregnancy team and inpregnancy team. The response to ovarian stimulation an d the pregnancy rates in different leptin/BMI (body mass index) levels teams wer e compared. Results The basal leptin and leptin/BMI levels were lower in pregnancy team. There were more mature oocytes retreived and the fertilization r ate, implantation rate, pregnancy rate were higher in low leptin/BMI levels team . The leptin/BMI>0.65 predicted significantly decreased pregnancy rates (P <0.05). Conclusion Basal FSH is a good index for predict the IVF-ET o utcome, elevated leptin levels particularly relative to BMI may impact the oocyt e and embryo quality, therefore the implantation rate decreased and the pregnanc y rate decreased either.
