尿胰蛋白酶原-Ⅱ测定在急性胰腺炎早期诊断中的价值

Role of urinary trypsinogen - II test in screening for early acute pancreatitis

目的 :探讨尿胰蛋白酶原 Ⅱ测定在急腹症中筛选急性胰腺炎的价值。方法 :对 15 7例连续就诊的急腹症病人同时测定尿胰蛋白酶原 Ⅱ及血、尿淀粉酶。结果 :39例急性胰腺炎中有 36例尿胰蛋白酶原 Ⅱ阳性 ,敏感性为 92 3% ,118例非胰腺炎的急腹症病人有 5例假阳性 ,特异性为 95 7% ,4例重症急性胰腺炎中尿胰蛋白酶原 Ⅱ全部阳性。结论 :尿胰蛋白酶原 Ⅱ是急腹症病人筛选急性胰腺炎快速而简便的方法 。

Objective: To evaluate the values of urinary trypsinogen - II test in screening for acute pancreatitis in patients with acute abdomen. Methods: The levels of urinary trypsinogen - II and serum and urinary amylases were assayed in 157 patients with acute abdomen, and the results were compared with one another. Results: The diagnosis of acute pancreatitis was established in 39 patients among 157 ones. Of 39 ones, 36 ones were positive to urinary trypsinogen - II and the sensitivity was 92.3%. Five cases with false positive were found in 118 ones of non - pancreatitis and the specificity was 95.7%. All four cases with severe acute pancreatitis were positive to urinary trypsinogen - II. Conclusion: Urinary trypsinogen - II test is a sensitive and specific method to screen acute pancreatitis in patients with acute abdomen.
