胃癌前病变组织和胃癌中hTERT的表达及其与幽门螺杆菌感染的关系

Relationship between hTERT expression in human gastric carcinoma and its precancerous lesions and Helicobacter pylori infection

目的 探讨胃癌前病变组织及胃癌中人端粒酶催化亚单位 (humantelomerasecatalyticsubunit,hTERT)的表达状况及其与幽门螺杆菌 (Helicobacterpylori ,Hp)感染的关系。方法 应用免疫组织化学技术检测 45例慢性胃炎和 19例胃癌中hTERT蛋白的表达 ,用快速尿素酶法和Warthin Starry银染色法检测Hp。结果 hTERT蛋白的表达率在胃癌组织( 78 95 % )中显著高于异型增生 ( 64 2 9% ,P <0 0 5 )、肠化生 ( 4 6 67% ,P <0 0 5 )和萎缩性胃炎组织 ( 2 5 0 0 % ,P <0 0 5 ) ,在异型增生组织 ( 64 2 9% )中显著高于肠化生 ( 4 6 67% ,P <0 0 5 )和萎缩性胃炎组织 ( 2 5 .0 0 % ,P <0 0 5 ) ,在肠化生组织( 4 6 67% )中显著高于萎缩性胃炎组织 ( 2 5 .0 0 % ,P <0 0 5 )。Hp阳性患者中hTERT蛋白的表达率为 77 42 % ,Hp阴性患者中hTERT蛋白的表达率为 3 3 3 3 % ,二者之间差异非常显著 (P <0 0 1)。结论 在胃癌和胃癌前病变阶段 ,端粒酶 (telom erase)均可能发挥重要作用 ,促进了胃癌的发生、发展。Hp可能是胃粘膜上皮细胞端粒酶激活的主要机制之一。

Objective To study the expression of hTERT in human gastric carcinoma and its precancerous lesions and to explore the relationship between hTERT expression and Helicobacter pylori (Hp) infection. Methods hTERT expression was detected by immunohistochemical method. Hp was detected by rapid urease test and Warthin-Starry silver staining. Results The expression level of hTERT in human gastric carcinoma (78.95%) was significantly higher than that in gastric dysplasia (64.29%), in intestinal metaplasia (46.67%), and in atrophic gastritis (25.00%, P<0.05). The positive rate of hTERT protein in patients with Hp infection (77.42%) was significant higher than that in patients without Hp infection (33.33%, P<0.01). Conclusion The results suggest that the activity of telomerase may play an important role in the genesis and progression of gastric carcinoma. Hp infection may be one of the mechanisms in the telomerase activation in the epithelial cells in gastric mucosa.
