摘要
目的 研究应用超声消融术治疗糖尿病足 (DF)的近期和远期疗效。 方法 对 6 6例DF患者进行下肢动脉造影 ,确定动脉闭塞的部位 ,采用经皮股动脉穿刺法或显露切开法插入超声消融导管 ,在血管造影监视下进行血管内超声消融。 结果 经 2周治疗 6 1例手术获得成功 ,5例失败 ,有效率达 92 4 %。 35例静息痛患者中 2 9例缓解 ,2 9例足趾坏死中 2 1例好转 ,长期随访 10例发生股浅动脉再闭塞 ,远期有效率达 83 3%。 结论 超声消融可以确切改善下肢动脉供血 ,治疗DF。但如何保证长期疗效有待于进一步研究。
Objective To evaluate the efficacy of the ultra so und ablations in the treatment of peripheral arterial occlusion of foot in d iabetic patients. Methods 66 patients with peripheral arteria l occlusion were treated with the ultrasound ablation. Through percutaneous app roach or surgical approach the ultrasound ablation catheter was inserted int o occluded arterial segments. Ultrasound frequency was 42 kHz and ultrasou nd energy was 45 watts. Results The ultrasound ablation was s uccessful in 61 of 66 patients (92.4%). After the treatment the gangrene was m itigated in 21 of 29 patients. The rest pain was improved in 29 of 35 patients . Long-term efficacy was 83.3%. Conclusion Ultrasound abla tion was effective and was a new approach for treatment of peripheral arterial occlusion in diabetic mellitus.
