急进高海拔地区健康男性青年动脉和静脉血浆降钙素基因相关肽等六种血管活性物质含量变化的研究

摘要 目的 探索健康人体急进高原低氧环境后 ,降钙素基因相关肽等 6种血管活性多肽的动态变化以及肺脏在其代谢中的作用。方法  5 0名健康青年男性志愿者根据海拔梯度和时相分为 :对照组 (海拔 1 1 0 0m)和缺氧组。缺氧组包括 :中度海拔地区 ( 2 2 60m) ,高海拔地区 ( 3780m) 1天、5天和 1 5天。采用放射免疫法 (RIA)测定 5组动脉和静脉血浆内皮素 (ET) ,降钙素基因相关肽 (CGRP) ,C型利钠肽 (CNP) ,神经肽Y(NPY) ,神经降压素 (NT)和β-内啡肽 ( β -EP)含量。结果  1 .6种因子含量方差分析结果 :①ET :高海拔 5天组动静脉血浆ET含量显著减少 (动脉血浆含量与对照组、中度海拔组和高海拔 1天组相比 ,静脉血浆含量与中度海拔组相比 ,P均 <0 .0 1 ) ;高海拔 1 5天组静脉血浆ET含量显著增加 (与对照组、中度海拔组、高海拔 1天组和 5天组比较 ,P <0 .0 1 )。②CGRP :中度海拔组动静脉血浆CGRP含量达峰值 (与对照组相比 ,P <0 .0 1 ) ,随后持续下降 ,高海拔 1 5天组降至谷底 (与对照组、中度海拔组、高海拔 1天组和 5天组相比 ,P <0 .0 1 )。③CNP :CNP动脉和静脉血浆含量随海拔升高明显增加 (高海拔 1 5天组与前 4组相比 ,P <0 .0 1 )。④β -EP :缺氧主要影响动脉β -EP含量 ,中度海拔组动脉血浆 β Objective To study effects of Calcitonin gene-related peptide(CGRP) , Endothelin(ET), C-type natriuretic(CNP) ,Neurotensin (NT), (β-endorphin,β-EP)and NeuropeptideY (NPY) on physiological regulation of pulmonary circulation and to probe the role of the lung in metabolism of these active polypeptides in lower altitude healthy volunteers at high altitude.Methods Volunteers were randomly divided into control group(those residing at 1 100m above sea level,CG) and hypoxic group [those traveling to 2 260m altitude ,(MA) and at 3 780m high altitude at 1d(H1d), 5d (H5d) and 15d(H15d)]. Arterial (A) and venous (V) blood samples were collected and were measured by radioimmunoassay. Results 1.Analysis of variance: ① ET The plasma ET content of A and V in H5d was significantly decreased (the content of A compared with CG, MA and H1d,P<0.01). In H15d the content of V increased significantly compared with the content of V in MA, H1d , H5d and that of A in H15d (P<0.01). ② CGRP The contents of CGRP in plasma of A and V in MA had reached peak (compared with CG,P<0.01),then they continuously dropped with increase of altitude and prolongation of time in hypoxia. The differences were significant in H15d compared with CG,MA,H1d and H5d (P<0.01). ③ CNP The CNP contents of A and V were significantly increased with increase of altitude and prolonged of period of time at high altitude. CNP content was remarkably higher in H15d than in CG,MA,H1d and H5d(P<0.01). ④ NT The A contents of NT reached a peak in H5d (compared with CG,MA and H1d,P<0.01)and The V contents was the highest in H15d (compared with MA , H1d and H5d,P<0.01). ⑤ NPY there were no significant of the NPY content between A and V and among groups. ⑥ β-EP The A plasma content of β-EP dropped significantly in MA(compared with CG,P<0.01 ), then it gradually decreased after reached peak in H5d (H5d compared with MA, H15d with H5d,P<0.01). 2. t- test of between A and V each group There were significant differences between A and V of ET and CGRP in H15d and NT in CG and H5d. 3.Rectification correlation analysis CGRP A and V plasma content versus ET showed negative correlation(r=-0.626 and -0.425,P<0.01和0.05)in H15d. CNP V plasma content versus and ET V plasma content showed positive correlation.Conclusion 1.ET, CGRP and CNP play a pivotal role in the physiological regulation of the pulmonary circulation after exposure to hypoxia. 2.Acting as a kind of nervous sending substance and circular hormone, NT and β-EP are involved in the physiological reaction to hypoxia. 3.CGRP and β-EP are involved in the hypoxia-extending-vessels regulatory reaction of pulmonary circulation at an altitude of 2 260m and 3 780m at early period. 4. ET plays no intermediate and introductory role in the urgent hypoxic contraction of pulmonary vessels. 5. The lungs, through the compound, release and cleanup of CGRP and ET, and the metabolism of NT, β-EP, keeps the dynamic balance among the various genes in regulatory network. 6. The climatic environment of the mediate altitude areas can start up the hypoxic physiological reaction of the human body.
