武威市世行贷款结核病控制项目对比性研究 被引量:1

摘要 目的通过不同时期结核病控制策略对比研究,评价世行贷款结核病控制项目成效及对传染源发现率的影响,分析存在的问题,探索项目策略、措施的可持续性.方法根据结核病控制项目实施前后各类报表及结核病流行病学抽样调查资料,对结核病人登记、治疗效果、成本效益及传染源发现率影响因素进行分析研究.结果武威市世行贷款结核病控制项目从1993年开始实施,9年间累计接诊可疑肺结核18 415例,发现涂阳肺结核3 915例,其中新发涂阳肺结核2 584例,复发及其他复治涂阳1 331例.与规划实施期相比发现的涂阳肺结核病例提高1.3倍.传染源发现率由项目实施前的34%,提高到69.5%,涂阳病例治愈率由项目实施前的77.4%,提高到95.4%.项目成本-效益比(CBR)1/18.4.结论武威市世行贷款结核病控制项目取得了明显的社会效益和经济效益.涂阳病人发现率及治愈率逐年提高符合结核病控制项目高发现率、高治愈率的要求,达到了控制传染源的目的,其主要原因是行政干预、加强结防宣传和DOTS策略的落实.
