AFP、AFU、GPDA-F联合检测对肝癌的诊断价值

Diagnostic value of Combined Detection of Serum AFP, AFU and GPDA-F in Primary Hepatic Carcinoma

目的 :探讨血清AFP、AFU和GPDA -F联合检测对肝癌的诊断价值。方法 :对 71例肝癌和 85例良性肝病(肝硬化、慢性肝炎 )患者的血清甲胎蛋白 (AFP)、α -L岩藻糖苷酶 (AFU)、甘氨酰脯氨酸二肽氨基肽酶同工酶 (GPDA -F)进行同步检测分析。结果 :肝癌组的AFP、AFU、GPDA -F的阳性率均明显高于良性肝病组 (P <0 .0 1) ;AFP、AFU和GPDA -F对肝癌的诊断敏感性分别为 70 .4%、62 .0 %和 77.5 % ,特异性分别为 82 .4%、87.1%和 84 7% ;肝癌患者血清AFP、AFU、GPDA -F之间无相关性 ,对肝癌有互补诊断价值 ;在AFP <40 0mg/L的 2 1例肝癌患者中 ,单项GPDA -F和AFU的阳性率分别为 71.4%与 47.6% ,联合检测的阳性率达 85 .7%。结论 :联合检测AFP、AFU和GPDA

ObjectiveTo investigate the value of combined detection of serum AFP, AFU and GPDA- F in the diagnosis of primary hepatic carcinoma (PHC).MethodsAFP, AFU and GPDA- F were measured simultaneously in 71 cases with PHC, 44 cases with liver cirrhosis and 41 cases with chronic hepatitis. The diagnostic value of combination assay of these indices was evaluated. ResultsThe positive rates of AFP, AFU and GPDA- F in cases with PHC were significantly higher than that in benign liver disease. The diagnostic sensitivities of AFP, AFU and GPDA-F for PHC were 70.4%, 62.0% and 77.5% respectively and the specificities were 82.4%, 87.1% and 84.7% respectively. AFP, AFU and GPDA-F were complement for the diagnosis of PHC. In 21 cases with PHC whose AFP were less than 400μg/L, the positive rates of GPDA-F and AFU were 71.4% and 47.6%, and reached 85.7% with combination assay of AFU and GPDA-F.ConclusionsThe combined assay of AFP,AFU and GPDA-F will be helpful in the diagnosis and differential diagnosis of PHC.
