解剖钢板治疗严重胫骨平台骨折

Treatment of Serious Tibia Plateau Fractures with Anatomical Bone Plates

目的 :探讨采用解剖钢板治疗严重胫骨平台骨折的疗效。方法 :回顾分析 3 5例严重胫骨平台骨折 ,按SchatzkerJ分型 ,Ⅴ型 12例、Ⅵ型 2 3例。全部患者均采用解剖钢板内固定 ,同时处理合并的半月板、交叉韧带、副韧带和血管损伤。结果 :本组患者均获随访 ,随访时间 6～ 2 0个月 ,平均 12个月 ;骨折愈合时间为 3～ 5个月。膝关节功能恢复按Hohl评分法进行评定 ,优良率达 82 .9%。结论 :准确评估伤情 ,关节面的解剖复位 ,采用解剖钢板做坚强的内固定 ,正确处理合并伤 。

ObjectiveTo study the treatment of serious tibia plateau fractures.Methods35 cases of serious tibia plateau fractures were treated from 2000 to 2003 in our hospital .According to Schatzker J classification ,type Ⅴ 12 cases, type Ⅵ 23 cases .All patients were treated with internal fixation with anatomical bone plates, the intercurrent injuries of meniscus , cruciate ligament, collateral ligament and popliteal vas were treated at the same time. ResultsAll cases were followed up for 6 to 20 months (averagely 12 months). The union time was 3 to 5 months. The excellent and good rate was 82.9% based on Hohl scale for knee joint.ConclusionsExact evaluation of the injuries, anatomical reduction of the articular surface, strong internal fixation with anatomical bone plates, correct treatment of the intercurrent injuries and early, right functional exercise will enhance the curative effects of the serious tibia plateau fractures.
