二维超声及彩色多普勒超声对肝结核的诊断价值

Value of two-dimensional and color Doppler ultrasonography in diagnosis of hepatic tuberculosis

目的 探讨二维及彩色多普勒超声在肝结核中的诊断价值。方法 对经病理和临床证实的 18例肝结核的声像图、病理及临床资料进行回顾性对比分析。结果 根据声像图表现 ,肝结核可分为三型 :弥漫型 (8例 ) ,实性团块型 (8例 ) ,脓肿型 (2例 )。结论 弥漫型可能为粟粒型肝结核的声像图特点 ,实性团块型为结核性肉芽肿伴干酪样坏死的声像图特点 ,脓肿型为干酪样坏死伴中心液化的声像图特点。

Objective To investigate the value of two-dimensional and color Doppler ultrasonography in diagnosing hepatic tuberculosis. Methods The results of ultrasonograpy, pathology and clinical data in 18 patients with hepatic tuberculosis were retrospectively analyzed. Results The ultrasonographic features were divided into three types: diffusive type (8 cases), solid lesion type (8 cases), and abscess type (2 cases). Conclusion Ultrasonography of diffusive type may be the characters of miliary hepatic tuberculosis. Solid lesion type can reflect characters of tuberculosis granulomatous and caseous necrosis, abscess type may be the characters of tuberculosis caseous necrosis with central liquefaction. Combining ultrasonographis appearance with clinical data would be useful in diagnosis of hepatic tuberculosis.
