摘要 目的 探讨应用动态增强磁共振成像 (DCE MRI)评价的孤立性脑转移瘤血管通透性与血管内皮生长因子(VEGF)表达的空间分布相关性。方法  2 4例孤立性脑转移患者于术前行DCE MRI检查 ,分析肿瘤不同区域强化斜率 ,测定标本VEGF表达。结果 肿瘤强化呈环形 16例 ,结节状 4例 ,靶心样 4例。在环形强化中 ,肿瘤边缘呈高通透性 ,边缘VEGF高表达 12例 ;在结节状和靶心样强化中 ,边缘与中心区均表现为高通透性 ,边缘VEGF高表达为 6例 ,中心区高表达为 5例。VEGF表达与DCE MRI强化斜率间具有显著的相关性 (P <0 .0 1)。结论 DCE Objective To evaluate the correlation between vascular permeability by dynamic-contrast enhanced MRI (DCE-MRI) and vascular endothelial growth factor (VEGF) expression in solitary brain metastasis. Methods Twenty-four patients with solitary brain metastasis underwent DCE-MRI examinations before operation. The slopes of the signal-time curve were calculated from the central, border and peripheral parts of tumors, respectively. The VEGF expression in tumors and surrounding tissues were determined with immunohistochemistry stain. Results Rim enhancement was found in 16 cases, while nodular and target enhancement was each found in 4 cases, respectively. In the rim enhancement pattern, the border of the tumor had high enhancement slope. In 12 cases, the VEGF expression was high in the border. In the nodular pattern and the target pattern, both the border and the central part had a high permeability. In 6 cases, the VEGF expression was high in the border and in 5 cases the VEGF expression was high in central part of the tumors. These results were highly significant ( P <0.01), suggesting that high VEGF expression in the tumors had a high enhancement slope. Conclusion The VEGF expression of brain metastases has a positive correlation with enhancement slope in DCE-MRI. DCE-MRI can be used in the surveillance of antiangiogenesis treatment of brain metastases.
