睾丸扭转术后随访分析

Postperative results in males treated for testicular torsion

目的探讨睾丸扭转术后转归及相关因素。方法对45例病例行病史回顾及随访。内容包括年龄、病程、扭转程度,睾丸体积,精液常规等。用χ2及确切概率法统计。结果全组睾丸获救率为42.2%,它与病程长短(P<0.005)扭转度数(P<0.025)有关。获访21例,时间8个月～12年。获救睾丸萎缩率66.7%,它也与病程长短(P=0.047)、扭转度数(P=0.017)有关。病程大于24h患儿睾丸全部萎缩。11例获精液常规检查,7例异常。结论扭转睾丸坏死和萎缩的相关因素病程和扭转程度。病程大于24h或扭转大于540°的睾丸几乎无存活可能。同时影响生精功能。

Objective To evaluate the outcome and risk factors in children with testicular torsion.Methods Forty five patients were followed up and the age of patients at torsion, duration of symptoms, testicular volume and semen analysis were retrospectively studied. The results were analyzed with chi square or exact probabilities in 2×2 table.Results The rate of testis salvage was 42.2% and it was related to the duration( P < 0.00 5) and degree of torsion( P < 0.025 ).Twenty one patients were followed up for 8 months to 12 years. The rate of secondary testicular atrophy was 66.7% and was related to the duration( P = 0.047 ) and degree of torsion( P = 0.017 ). All patients with history more than 24 hours had atrophy. Eleven patients had semen analysis performed and seven were abnormal.Conclusions The rate of salvage and atrophy was related to the duration and degree of torsion found at surgical detorsion of the testis. In those torsions lasted for more than 24 hours, the viability of the testis is questionable and subsequent fertility is affected.
