摘要
立体定向放射外科是近年发展起来的边缘学科,圆满完成临床工作需多学科密切合作。立体定向放射外科是脑转移瘤安全有效的治疗方法。本文介绍了肿瘤放射生物学基础相关知识,放射损伤概念:亚致死损伤,潜在致死损伤,致死损伤;早、晚反应组织的不同生物效应;立体定向放射外科治疗靶区的4种情况;肿瘤放射生物学的4个 R:细胞放射损伤的修复(repair of radiation damage).周期内细胞的再分布(redistribution withinthe cell cycle),氧效应及乏氧细胞的再氧合(oxygen effect and reoxygenation)和再群体化(repopulation)。提出分次立体定向放射治疗的优越性。复习国外系列报道,分别从治疗适应证的选择.是否配合全脑放射,作为复发或未控的挽救治疗,治疗多发转移瘤和立体定向放射外科的疗效评价方面进行综述。
In this presentation,emphasis is laid upon stereotactic radiosurgery(SRS),a frontier science developed within the recent years.It has greatly encouraged and enhanced the cooperation among many scientific domains.Knowledge about the basic concepts related to tumor radiobiology,radiation damage:sublethal damage, potential lethal damage and lethal damage itself,together with the immediate and late biological responses in the affected tissues are discussed.Explanation of the 4 types of target area,the 4 Rs of radiobiology:Repair of radia- tion damage,redistribution within the cell cycle,oxygen effect and Reoxygenation and Repopulation are also made.The conclusion of being safe and effective are arrived at.The superiority of fractionated SRS is unescapably elucidated.Review of literature published abroad examplifies the indications,whether or not to be combined with whole-brain irradiation and the salvage treatment of recurrenrce and un-control,treatment of muhple metastatic foci and the evoluation of result of SRS are involved in the close of presertation.
