结肠癌手术后患者生活质量的评估

Assessment of quality of life in patients after colon carcinoma operation

目的:评价手术对结肠癌患者生活质量的影响。方法:2000-01/2003-01沈阳铁路局中心医院普外科住院治疗的结肠癌患者72例。纳入经病理确诊为结肠癌且无远处或全身转移患者。排除合并有其他严重躯体性疾病和精神症状或精神病家族史者。符合标准患者65例。采用胃肠生活质量量表(gastrointestinalqualityoflifeindex,GIQLI)评定65例结肠癌手术患者术前及术后1,6,12个月的生活质量。结果:结肠癌患者手术后1个月平均GIQLI评分犤(92.26±10.67)分犦明显低于手术前犤(106.32±11.29)分犦(t=35.203,P<0.01)。术后12个月平均GIQLI评分犤(114.05±8.2)分犦明显高于术前(t=-9.549,P<0.01)。结论:结肠癌手术近期患者生活质量下降,与手术创伤和手术并发症有关,手术远期患者的生活质量明显提高。

AIM:To evaluate the effect of operation on quality of life(QOL) in patients with colon carcinoma. METHODS:Totally 72 inpatients with colon carcinoma in the Department of General Surgery,Central Hospital of Shenyang Railway Bureau from January 2000 to January 2003.The patients,who were diagnosed to have colon carcinoma by pathology;had no distant or total body transferring were selected;while those had serious somatization disease with complications,psychiatric symptoms or history were excluded.Totally 65 patients were included.The gastrointestinal quality of life index(GIQLI) was used to assay the QOL of 65 patients before and 1,6,12 months after colon operation. RESULTS:The mean preoperative GIQLI score of these patients after 1 month( 92.26±10.67) was significantly lower than that before operation(106.32±11.29)(t=35.203,P< 0.01).The mean preoperative GIQLI score of these patients after 12 months(114.05±8.2)was significantly higher than that before operation(t=-9.549,P< 0.01). CONCLUSION:The QOL of patients with colon carcinoma reduces after operation in a short term because of the damage of operation and complications,and it significantly improved after operation in a long term.
