中医辨证施治对肺癌术后患者心理健康及生活质量的影响

Effects of syndrome differentiation of traditional Chinese medicine on mental health and the quality of life of the patients with lung cancer after operation

目的:评价中医辨证施治对肺癌术后患者心理健康及生活质量的影响。方法:解放军第四军医大学唐都医院收治的26例肺癌术后患者,在常规化疗的基础上,加服中药,采用精神卫生症状自评量表(SCL-90)和癌症患者生活质量问卷(QLQ-C30)量表观察肺癌术后患者治疗后心理健康及生活质量的改善情况。结果:治疗2个疗程后,SCL-90各大项评分(包括躯体化、强迫、人际关系、抑郁、焦虑、敌对、恐怖、偏执、精神病性)明显低于治疗前(P<0.05或P<0.01),QLQ-C30量表(内容包括呼吸困难、疲乏、恶心呕吐、食欲缺乏、失眠、疼痛、便秘、经济困难)显著高于治疗前(P<0.05或P<0.01),血清IgA,IgG,IgM治疗前的值为(1.25±0.23),(9.67±2.76),(0.89±0.43)g/L,治疗后则为(1.41±0.27),(11.42±3.02),(1.24±0.59)g/L,较治疗前比,亦有显著改善(P<0.05)。结论:中医药治疗可改善肺癌术后患者的精神心理状态和生活质量,提高免疫调节功能。

AIM:To evaluate the effects of syndrome differentiation of traditional Chinese medicine(TCM) on mental health and the quality of life(QOL) of the patients with lung cancer after operation. METHODS:Twenty six patients with lung cancer after operation hospitalized in Tangdu Hospital,Fourth Military Medical University of Chinese PLA were treated with TCM based on the conventional chemiotherapy.The symptom checklist 90(SCL 90) and EORTC core quality of life questionnaire(QLQ C30) were used to assess mental health and the QOL. RESULTS:After two courses of treatment, the scores of SCL 90(concluding somatization,compulsion,personal relationship,depression,anxiety,rivalry,panic,paranoid and phychosis) were significantly lower than those before treatment(P< 0.05 or P< 0.01).The scores of QLQ C30(including dyspnoea,tiredness,nausea and vomiting,anorexia,agrypnia,pain,constipation and financial straits) were significantly higher than those before treatment(P< 0.05 or P< 0.01),the values of blood serum IgA,IgE and IgM were (1.25±0.23),(9.67±2.76) and(0.89±0.43) g/L respectively before treatment,and (1.41±0.27),(11.42±3.02) and (1.24±0.59) g/L,respectively after treatment,and they were improved obviously compared with that before treatment(P < 0.05). CONCLUSION:TCM treatment can improve the mental status and the QOL of the patients with lung cancer after operation,and can promote the immunoloregulational function.
