彩超检查在颌面部复杂缺损组织瓣修复中的应用

Use of Color Doppler for Reconstruction of Complicated Defection with Free Flap at Oral and Maxillofacial Region

目的 :探索彩超检查在颌面部复杂缺损畸形组织瓣修复中的作用。方法 :对 3例曾因外伤或经手术而存在明显瘢痕组织、已经存在或进一步手术将会出现严重缺损畸形的患者 ,运用acuson12 8/×p10型彩色超声检测仪检测受区及皮瓣的血供状况 ,根据检查结果制定治疗方案 ,然后进行组织瓣 (前臂皮瓣和腓骨肌皮瓣 )转移修复手术。结果 :彩超检查结果与术中所见相吻合 ,术后组织瓣全部存活。结论 :彩超检查无创伤、易被患者接受 ,结果准确、可靠 ,比较适合于头颈部组织瓣转移时血供状况的检测 ,应该作为颌面部复杂畸形皮瓣修复的常规术前检查方法之一。

Objective: We assessed the utility of color doppler for reconstruction of complicated defection with free flap at oral and maxillofacial region. Methods: 3 patients had been operated or traumatized before. So there were surgical scars at oral and maxillofacial region. These patients had or would have complicated defections to be reconstructed with free flap (forearm flap and osteomyocutaneous flap of fibular). Acuson128/×p10 color doppler examination were performed on these 3 patients for assessing patent and diameter of desired vessels at donor and recipient site with color doppler imaging preoperatively. Results: The results of color doppler imaging were correspondent to what were showed at surgery. All the 3 cases had successful flap transfer. Conclusion:Color doppler examination is undamaged and liable to be accepted by patients. It's a useful technique in microsurgical tissue transfer for assessing patent and diameter of desired donor and recipient vessels preoperatively, especially for the patient with unstable vascular location.
