AO跟骨钢板治疗Sanders Ⅳ型跟骨骨折

Treatment for Sanders type Ⅳcalcaneal fracture with AO-calcaneal plates

目的总结用AO跟骨钢板治疗SandersⅣ型跟骨骨折的经验。方法对15例18足SandersⅣ型跟骨骨折用AO跟骨钢板进行开放复位内固定治疗,采用Kerr设计的跟骨骨折百分评分系统作为疗效评定标准。结果18足骨折术后全部得到随访,时间7～39个月,平均11.6个月,优良率83.3%。手术并发症发生率为16.7%,以前期为主,主要是皮缘坏死。结论开放复位AO跟骨钢板内固定是治疗SandersⅣ型跟骨骨折的良好方法。临床治疗效果与跟骨后距下关节面、跟骨的Bhler氏角、Gissane角以及跟骨的长、宽、高等的复位质量有关。

Objective To sum up the experiences of surgical treatment of Sanders type Ⅳcalcaneal fracture with AO calcaneal plates. Methods 18 feet of 15 patients with Sanders type Ⅳcalcaneal fracture underwent open reduction and internal fixation with AO calcaneal plates. The results were evaluated according to the Kerr scoring criteria. Results All the 18 feet were followed up for a mean of 11.6 months (ranging from 7 to 39 months). The excellent and good rate was 83.3%. And the complication rate was 16.7%. Most complications occurred at the early stage and in the form of necrosis of skin edge. Conclusions Open reduction and internal fixation with AO calcaneal plate is a good procedure for inter articular calcaneus fractures. The clinical outcome is related to the reduction of posterior subtalar joint, Bhler angle, Gissane angle, as well as the height, width and length of the calcaneus.
