脑电图反应性对急性重型脑伤患者预后的评估研究

A study of prognostic value of EEG reactivity on comatose patients with acute severe head injury

目的 探讨脑电图(EEG)反应性对急性重型脑伤患者预后的评估价值。方法 选用数字化脑电监测系统,对66例急性重型脑伤患者在72小时内行EEG监测,将EEG的反应性和患者开始的GCS评分与其预后结果进行比较分析。结果 EEG反应性对本组急性重型脑伤患者预后评估的准确率(95t45％)、敏感性(97.05％)、特异性(93.75％)均高于其GCS评分(71.21％,56.75％,89.65％);本组EEG反应性错误率为3.03％,低于其GCS评分错误率(4.55％)。结论 EEG反应性对急性重型脑伤昏迷病人预后评估有较高的价值。

Objective To reveal prognostic value of EEG reactivity on comatose patients with acute severe head injury. Methods Sixty-sk comatose patients with acute severe head injury were monitored by digital-EEG for 72 hours, compared the results of EEG reactivity and initial GCS with the prognosis of patients. Results The rate of accuracy, sensibility, specificity of EEG reactivity in prognosis prediction were 95.45% ,97.05% ,93.75% respectively, all of which were higher than that of GCS(71.21% ,56.75% ,89.65%). The error rate of EEG reactivity in prognosis prediction was 3.03% .which was lower than lhat of GCS(14.28%). Conclusion The results showed that EEG reactivity was an excellent predictor on the prognosis comatose patients with acute severe head injury.
