人工晶状体植入术后视网膜脱离的手术治疗

Vitreoretinal surgery for retinal detachment after intraocular lens implantation

目的 探讨人工晶状体植入术后视网膜脱离的手术方法并评价其疗效。方法 2 0 0 0年 1月～ 2 0 0 2年 6月经玻璃体视网膜联合术治疗人工晶状体植入术后视网膜脱离 3 1例 (3 1眼 )的临床资料作回顾分析。手术采取玻璃体切除、剥离视网膜前膜、激光封闭视网膜裂孔及眼内长效气体或硅油填充等方式。结果 随访期为 12～ 3 9月 ,平均 2 2月。随访时视网膜复位 2 9眼 ,占 93 5 5 %。有 2 7眼 (占 87 11% )视力提高 ,其中≥ 0 1者 2 2眼 ,最好视力为 0 8。结论 人工晶状体植入术后视网膜脱离使眼内病变复杂 ,易致严重PVR。应用玻璃体视网膜联合术 ,可使视网膜得以有效复位。

Objective To evaluate the therapeutic effect o f vitreoretinal surgery for retinal detachment(RD) after intraocular lens(IOL)impl antation.Methods Retrospectively study the 31 patients(31 eyes) of retin al detachment after intraocular lens implantation from January,2000 to June,200 2 which underwent vitreoretinal surgery.Results The follow-up period was 12 to 39 months with the avera ge period of 22 months.At the final examinations,the retinas were attacthed in 2 9 eyes(93.55%),the visual acuity improved in 27 eyes(87.11%),and reached 0.1 or better in 22 eyes,the best visual acuity was 0.8.Conclusion Retinal detachment after IOL implantation is more s ever than general RD.With vitreoretinal surgery,the tetinas can be effctively re attached and the visual acuity can be protected and reobtained.
