白内障术中后囊浑浊的处理

Management of the posterior capsular opacity during ECCE with IOL implantation

目的 评价白内障术中后囊浑浊的处理方法和临床意义。方法 术中发现后囊浑浊 2 9眼 ,分别行后囊连续环形撕囊 (PCCC)、后囊三角形切除及后囊开窗嵌顿。部份病例联合前部玻璃体切除。结果 术后全部术眼视轴透明 ,人工晶状体在位。术后视力 0 1～ 0 2者 4眼 ,0 3～ 0 5者 12眼 ,大于 0 6者 13眼。术中并发症主要为玻璃体脱出 (10眼 )。术后并发症为黄斑囊样水肿 (3眼 )。结论 术中发现的后囊浑浊应Ⅰ期采用不同术式 ,行PCCC或后囊切除。

Objective To evaluate the methods of managemen t and their clinical significance for the posterior capsular opacification (PCO) d uring ECCC with IOL implantation.Methods There were 29 cases (2 9 eyes) with PCO were found during ECCE with IOL implantation.Among them 19 eyes were undergone by posterior continuance circular capsulorhexis (PCCC),5 eyes by triangular posterior capsulectomy and 5 eyes by opening window posterior capsul tomy with their lamellas were embed between the inferior optic of IOL and poste rior capsule.Among them 8 eyes combined anterior vitrectomy.Results The visual axis were clear and IOLs in position in all operation eyes.The v isual acuity was 0.1-0.2 in 4 eyes,0.3-0.5 in 12 eyes and better than 0.6 in 13 eyes.The main complication was vitreous loss (10 eyes) intra-operation and ma cular cystic edema (3 eyes)post-operatively.Conclusion The PCO that are found intra-operation should be undergone by PCCC or posterior capsulec tomy at the same time.It will avoid the trouble of secondary YAG laser treatment or operation for posterior capsulectomy post-operatively and save the patients treatment cost.
