电视腹腔镜下辅助子宫全切术30例临床探讨

The analysis of 30 patients Laparoscopiclly Assisted Vaginal Hysterectomy

目的通过对30例患者行腹腔镜辅助子宫全切术分析,进一步探讨腹腔镜辅助子宫全切术代替经腹子宫全切术的可能性、腹腔镜辅助子宫全切术的手术要点及临床应用价值。方法我院2002年3月至2004年3月腹腔镜子宫次全切除术30例。对30例因各种良性妇科疾病而需行子宫次全切除术的患者,采用腹腔镜辅助子宫全切术式。患者平均年龄44岁(39～68岁),术前子宫正常大小者6例,子宫增大、小于孕12周19例,大于孕12周5例,其中有下腹手术史者6例。结果30例行腹腔镜子宫次全切除术中,1例中转开腹手术,中转率3.33%。平均手术时间100分钟(60～240分钟),出血量约50～350ml。平均住院日为6天。结论腹腔镜辅助子宫全切术基本可代替经腹子宫全切术,可避免开腹、减少手术创伤。

Objective Though the analysis of 30 patients Laparoscopiclly Assisted Vaginal Hysterectomy. To evaluate the possibility that the Laparoscopiclly Assisted Vaginal Hysterectomy replaces the Laparoscopiclly Assisted Vaginal Hysterectomy. The technical procedure and the clinical value of Laparoscopiclly Assisted Vaginal Hysterectomy. Methods March 2002 to March 2004, 30 Laparoscopiclly Assisted Vaginal Hysterectomy in our hospital. Sixty patients suffered from various kinds of benign gynecological diseases underwent Laparoscopiclly Assisted Vaginal Hysterectomy. The mean age was 44 years (range 39～68),preoperative examination findings were 6 cases of normal uterine size, 30 of uterine enlargement (19 less than 12 week gestational size and 5 larger). Among these patients, 6 cases had a history of lower abdominal laparotomy. Results 1 out of the 30 cases were switched to laparotomy (3.33%). Mean operation time of Laparoscopiclly Assisted Vaginal Hysterectomy was 100 min (range 60～240min). The amount of blood loss was approximately 50～350ml during operation, and the average hospitalization was 6 days. Conclusion The Laparoscopiclly Assisted Vaginal Hysterectomy replaces the Laparoscopiclly Assisted Vaginal Hysterectomy basically, avoids laparotomy and lessns injuries.
