腹部小横切口改良式筋膜内子宫全切术的临床应用研究

Clinically study of modified intrafascial hysterectomy with small-incision

目的探讨腹部小横切口改良式筋膜内子宫全切术的手术方法、适应症、术后疗效及临床应用价值。方法回顾性分析比较腹部小横切口改良式筋膜内子宫全切术88例(观察组)和传统子宫全切除术102例(对照组)患者的手术情况和术后情况。结果两组间手术时间、术中出血量、住院时间、术后阴道残端感染率及恢复期阴道出血率等有显著性差异(p<0.05),随访结果显示观察组患者术后腹部切口愈合情况、阴道保真度及性生活满意度明显优于对照组。结论腹部小横切口改良式筋膜内子宫全切术操作简单、创伤小,患者术后恢复快、有美容效果,性生活影响小;但要注意手术适应症的选择。

Objective To explore the operative method, indication, post-operative effects and clinically value of modified intrafascial hysterectomy with small-incision. Methods 88patients operated modified intrafascial hysterectomy with small-incision and 102 patients performed classified hysterectomy were reviewed. Results There are statistically significant differences in operating time, bleeding volume, the time of flatus and the hospital stay between two groups; Following-up investigation shows that there are also statistically significant differences in the abdominal incision scar, vaginal and cervical change and sexual function between tow groups. Conclusion Modified intrafascial hysterectomy with small-incision provides a simple, minimally invasive, quickly recovery and cosmetology effects operative methods, it almost have not effect on sexual function, however, the choice of operative indication should be pay more attention.
