电子阴道镜与病理学检查早期诊断宫颈癌

Clinical study of electronic colposcopy and pathological examination in the diagnosis of early cervical cancer lesions

目的 探讨采用电子阴道镜与病理细胞学检查早期诊断宫颈癌的临床价值。方法 应用电子阴道镜观察具有宫颈病变患者 35 0例 ,对可疑病例实施宫颈电环切活检术 15 8例 ,并对其细胞学结果与电子图谱作对比分析。结果 电子阴道镜发现宫颈上皮内瘤变 (CIN) 32例 ,宫颈癌 10例 ,宫颈人乳头瘤病毒(HPV)感染 12例 ,慢性宫颈炎 2 95例 ,宫颈肌瘤 1例 ;与病理检查比较CIN符合率 76 7% ,宫颈癌与HPV感染病例符合率为 10 0 % ,慢性官颈炎 94 1%。结论 对生育年龄的妇女定期行电子阴道境检查 ,并对可疑病变部位进行电环切活检能够早期而准确地发现宫颈癌及癌前病变。

Objective To explore the clinical value of electronic colposcopy and pathological cytology in the diagnosis of early cervical cancer.Methods 350 cases of cervical lesion patients were observed with electronic colposcopy,158 cases of suspicious patients were performed with biopsy of loop electrosurgical excision procedure(LEEP);the results of cytology and electronic atlas were analyzed.Results The electronic colposcopic diagnosis was:34 cases with cervical intraepithelial neoplasia(CIN),11 cases with cervical cancer,14 cases with human papillomavirus(HPV) infection,295 cases with chronic cervicitis,1 case with cervical myoma.The pathological examination showed 23 cases with CIN,the coincidence rate of diagnosis was 76.7%;the coincidence rate of cervical cancer and HPV infection were 100%,chronic cervicitis 94.1%.The high incidences of CIN and HPV infection were found among women from 20 to 40 years old.The high incidence of cervical cancer was found among women from 30 to 50 years old.Conclusion Women with childbearing age should receive electronic colposcopic examination regularly,it can find early and accurately the cervical cancer and CIN or HPV infection with biopsy of LEEP.It has an important significance to prevent and treat cervical cancer.
