肿瘤标志物联合检测及非线性映射在肺癌组织分型及病情评估的价值

摘要 目的 运用非线性映射技术 ,评价血清癌胚抗原 (CEA)、糖类抗原 12 5 (CA12 5 )、胃泌素、神经元特异性烯醇化酶 (NSE)水平对肺癌病情评估和组织分型的临床价值。方法 采用放射免疫分析法测定 5 1例肺癌患者血清CEA、CA12 5、胃泌素、NSE水平 ,并观察了这些患者血清该 4项肿瘤标志物水平在不同的组织分型、不同的恶性肿瘤国际临床病期 (TNM)分期以及治疗前后的变化 ;同时采用非线性映射技术 ,探讨该 4项肿瘤标志物在肺癌组织分型中的价值。结果 小细胞肺癌 (SCLC)患者胃泌素、NSE水平显著高于非小细胞肺癌 (NSCLC)患者 ,而CEA、CA12 5水平却明显低于NSCLC患者 ;TNM分期越晚 ,CEA、CA12 5、胃泌素、NSE水平越高 ,其增高的程度与TNM分期有一定的关系 ;化疗前后该 4项肿瘤标志物水平差异有统计学意义 ,化疗后呈现明显下降 ,总有效率为 4 5 .0 9% ;非线性映射判别SCLC与NSCLC总的符合率为 94 .1%。结论 该 4项肿瘤标志物联合检测在肺癌组织分型、病情监测和疗效判断方面可为临床提供有价值的参考资料 ;此外非线性映射技术在SCLC与NSCLC判别中有一定的参考价值。 Objective To investigate the value of detection of four tumor markers (CEA, CA125, gastrin, NSE) for histological type and state of patients with lung cancer by non-linear mapping. Methods The levels of CEA, CA125, gastrin and NSE were detected by radioimmunoassay in 51 patients with lung cancer. The changes of four tumor markers were observed in different histological type, in different TNM period, before and after treatment. Non-linear mapping method was used to distinguish small cell lung cancer(SCLC) from non-small cell lung cancer (NSCLC). Results The levels of gastrin and NSE in SCLC were apparently higher than those of gastrin and NSE in NSCLC, but levels of CEA and CA125 in SCLC were significantly lower than those in NSCLC. The higher the levels of four tumor markers were, the later the TNM period was, and the elevation was consistent with TNM. The levels of four tumor markers after treatment were lower than those before treatment. Conclusions Combined detection of these four tumor markers could provide useful information for histological type, monitoring state of an illness and estimation of curative effect. And it was shown that non-linear mapping was a good method in distinguishing SCLC from NSCLC.
