老年人脉压指数与冠状动脉病变的关系

The relationship between pulse pressure index and coronary artery lesions

目的 :探讨脉压指数与冠状动脉病变严重程度的关系。方法 :82 0例均经冠状动脉造影检查 ,外周肱动脉压力测定收缩压 (SBP)、舒张压 (DBP) ,冠状动脉病变的严重程度用冠状动脉病变的血管支数表示。分别以脉压≤ 6 0mmHg(1mmHg =0 .133kPa)、>6 0～ 80mmHg、>80～ 10 0mmHg及 >10 0mmHg和脉压指数≤0 .4 0 0、>0 .4 0 0～ 0 .5 0 0、>0 .5 0 0～ 0 .6 0 0、 >0 .6 0 0 4个组进行分析。结果 :随着脉压和脉压指数的增加 ,冠心病患病率均随之增加 (P <0 .0 5 ) ,年龄呈增大趋势 ,双支和 3支冠状动脉病变患病率显著增高。Logistic回归分析 ,年龄、脉压、脉压指数对冠状动脉病变有最好的预测价值。结论 :脉压和脉压指数是最为方便的反映大动脉弹性的参数 ,与冠状动脉病变的严重程度密切相关 。

Objective:To investigate the relationship between pulse pressure index(PPI) and coronary artery lesions. Method: A total of 820 elderly patients were divided into 4 groups according to the level of pulse pressure(PP) [≤60 mmHg(1 mmHg= 0.133 kPa),61～80 mmHg, 81～100 mmHg and>100 mmHg]and PPI (≤ 0.400, 0.401～ 0.500, 0.501～ 0.600 and > 0.600). Brachial systolic and diastolic blood pressure were measured.The severity of coronary heart disease was indicated as the number of diseased arteries. Result:As PPI and PP increased,the incidance of coronary heart disease and ≥2 coronary artery disease went higher and age increased.In a multivariate analysis,age, PPI and PP were the best predictors for the severity of coronary lesions. Conclusion: PPI and PP are the simpliest paranmeter reflecting large artery elasticity and closely related to severity of coronary artery disease. PPI is better than PP in elderly patients.
