社会心理因素对老年消化性溃疡预后的影响

Effects of Psychosocial Factors on Prognosis in the Elderly Patients with Peptic Ulcer

目的 :探讨社会心理因素对老年消化性溃疡预后的影响。方法 :采用症状自评量表、生活事件量表、社会支持评定量表、吸烟问卷和饮酒问卷对老年消化性溃疡的 3期病例 (活动期、愈合期、疤痕期 )患者各 4 0例进行测试和评定。结果 :活动期病例 SCL- 90中的躯体化、人际关系、抑郁、焦虑等因子分值较高 ,有统计学意义 (P<0 .0 5～ 0 .0 1) ,在负性事件刺激量、生活事件总刺激量、主观支持、对支持利用度和总分、吸烟评分上 3组病例之间也有显著性差异 (P<0 .0 5～ 0 .0 1)。结论 :不良心理因素、负性生活事件刺激、社会支持程度下降以及吸烟是老年消化性溃疡难以愈合的危险因素 。

Objective:To explore the effects of psychosocial factors on the prognosis of peptic ulcer in the aged patients.Methods:The aged patients with peptic ulcer in the three stages (active,healing,scarring stages,40 cases each) were studied with Symptom Check List-90 (SCL-90),Life Event Scale (LES),Social Support Rating Scale (SSRS),Russell's Reasons for Smoking Questionnaire (RRSQ) as well as Alcohol Use Questionnaire (ADS).Results:The scores of the somatization,interpersonality,depression and anxiety in active stage were significantly higher than those in other two stages (P<0.05～0.01);The scores of poor and total life events,subjective support,supportive utility and smoking also showed significant difference between three stages(P<0.05～0.01).Conclusions:The harmful psychological factor,stimulus of poor life events,reduction of social support and smoking were the dangerous factors that caused peptic ulcer difficultly to heal,and also the important psychological factors to affect the prognosis of aged patient with peptic ulcer.
