摘要 目的 评价VITEKGPS 10 7药敏卡检测高浓度氨基糖苷类抗生素耐药肠球菌 (HLAR)的可靠性。方法 分别采用VITEKGPS 10 7药敏卡、脑心浸液琼脂法 (BHIA)对 112株临床分离到的肠球菌进行高浓度庆大霉素及高浓度链霉素耐药的检测 ,并以BHIA法为标准 ,评价其检测的准确性。结果  112株肠球菌中 ,BHIA法检出耐高浓度庆大霉素的有 5 1株、敏感的有 6 1株 ,VITEKGPS 10 7卡检测结果与BHIA法完全一致 ,敏感性和特异性均为10 0 % ;BHIA法检出耐高浓度链霉素的有 5 5株、敏感的有 5 7株 ,5 5株耐药株中VITEKGPS 10 7卡检测耐药 5 2株 ,敏感性为 94 5 % ,5 7株敏感株中VITEKGPS 10 7卡检测结果一致 ,特异性为 10 0 % ;3株BHIA法为耐药而VITEKGPS卡为敏感 ,分别是 1株粪肠球菌、2株屎肠球菌 ;3株鸟肠球菌两种检测法均为敏感。结论 VITEKGPS 10 7药敏卡检测肠球菌对高浓度庆大霉素及高浓度链霉素的耐药性具有操作简便、快速 ,结果准确性高等优点 。 OBJECTIVE To evaluate the ability of VITEK Gram positive Susceptibility 107 Card(GPS 107) to detect high level aminoglycoside resistance in enterococci. METHODS A total of 112 strains of enterococci were isolated from various clinical specimens. The resistance of 112 isolates to high level gentamicin and streptomycin was determined by VITEK system with GPS 107 Card and the Brain Heart Infusion Agar Screen method(BHIA). BHIA was used as a reference method to evaluate the results with GPS 107 Card. RESULTS Fifty one isolates were high level gentamicin resistant and 61 isolates were high level gentamicin susceptible by BHIA method and GPS 107. The specificity and sensitivity of GPS 107 were all 100%. Fifty two of 55 high level streptomycin resistant isolates determined by BHIA were positive by GPS 107. All 57 streptomycin susceptible isolates were determined correctly by GPS 107. Their specificity and sensitivity in determining high level streptomycin resistant isolates were 100% and 94 5%,respectively Three isolates determined falsely as streptomycin susceptible by GPS 107 were 2 Enterococus faecium and 1 E. faecalis. CONCLUSIONS VITEK GPS 107 is reliable for determining high level aminoglycoside resistance in enterococci.
