不同生理阶段女性下尿路感染治疗的研究

Management of female lower urinary infection using different ways in different age patients

目的 :研究不同生理阶段女性下尿路感染的特点 ,采用不同方法进行治疗 ,比较治疗效果。方法 :按不同生理阶段分为绝经组、青春期及生殖期妇女组、青春前期组。绝经组又分为单独使用抗生素及抗生素加雌激素制剂组 ,比较两组治疗效果。青春期及生殖期妇女组分为单独使用抗生素组及根据检查结果行阴道用药同时治疗泌尿系感染组 ,比较两组治疗效果。青春前期组针对病因进行治疗。结果 :绝经期组中加用雌激素制剂的治疗效果 (96 .2 % )明显优于单独使用抗生素组 (76 .0 % )。青春期及生殖期妇女组中阴道用药同时治疗泌尿系感染组的效果 (98.4 % )明显优于单独使用抗生素组 (6 7.4 % )。青春前期组 90 .3%治愈。结论 :不同生理段的女性下尿路感染病因不同 ,针对病因采用不同的治疗方法 ,有助于提高治疗效果。

Objective:To investigate the significance of using different ways to management of female lower urinary tract infection(UTI) in different age.Methods:A total of 311 cases of female lower UTI which aged 5 to 78years (averaged 47.7 years). were divided into three groups: ①postmenopausal; ② puberty; ③ prepuberal. In postmenopausal cases, lower UTI management were divided two types: antibiotic alone and used antibiotic plus Livial( 2.5 mg, Qd), the results of two groups were compared. In puberty cases, management was also divided two types: UTI treatment alone and UTI plus colpitis treatment the results of two groups were compared. In prepuberal, we seek the cause carefully.Results:311 cases included postmenopausal 129, puberty 151 cases, prepuberal 31. In group of postmenopausal patients which used antibiotics plus Livial, the lower UTI cure rate was 91.3%, improvement 4.8%, while in patients which used antibiotics alone, the cure rate was 48%, improve ment 28% (P< 0.01). In puberty cases with UTI plus colpitis, the lower UTI cure rate was 87.1%, improvement 11.3%. While in patients who cured UTI alone the cure rate was 43%, improve 25% (P< 0.01). In prepuberale patients, while the cause of lower UTI was eliminated, the cure rate was 90.3%, improve 9.7%.Conclusions:In postmenopausal women with lower UTI, management should use antibiotics plus estrogen at the same time. In puberty women, it is necessary to treat lower UTI plus colpitis at the same time. In girls, it is most important to eliminate the cause of lower UTI.
