摘要
目的 观察重症急性胰腺炎 (severeacutepancreatitis ,SAP)模型血浆淀粉酶、内毒素、IL 1β、IL 6、肺、胰腺的变化及清胰Ⅱ号对其的保护作用。 方法 用 16只贵州香猪 ,雌雄不拘 ,随机分为 2组 ,各组n =8:生理盐水 (NS)组 ;中药清胰Ⅱ号 (ChineseMedicineQingyiⅡ ,CMQ)组。推注 5 %牛磺脱氧胆酸钠 (0 .5ml/kg体重 )复制SAP模型。分别于制模前 ,制模后 1、6、12、2 4、4 8、72、96h采静脉血 ,检测血浆中淀粉酶、内毒素、IL 1β、IL 6浓度。光镜下 ,观察胰、肺组织的病理变化。 结果 CMQ可减轻SAP猪的胰腺及肺的病理性损伤 ,可减轻胰出血、坏死程度和减轻肺间质水肿、肺泡毛细血管壁缺血、缺氧程度 ,显著降低各时间点 (除制模前、制模后 1小时外 )血浆内毒素浓度和血清淀粉酶、IL 1β、IL 6的浓度 (P <0 .0 1)。结论 清胰Ⅱ号可减轻内毒素血症 ,抑制内毒素对全身各器官、组织的损伤 ;减少早期炎症介质IL 1β、IL 6的产生 ,减轻其引发的一系列损伤反应。对胰肺有保护作用。
Objective To investigate the mechanisms of Chinese Medicine Qingyi tang on miniature pigs suffering from severe acute pancreatitis (SAP) complicated by endotoxemia. Methods 16 Guizhou miniature pigs were randomly divided into 2 groups (n=8), including control group (NS) and Chinese Medicine Qingyi tang group (CMQ). SAP models were induced by injecting 5% sodium deoxycholate (0.5 ml/kg weight). Before and 1 h, 6 h, 12 h, 24 h, 48 h, 72 h, 96 h after inducing of SAP, blood samples were taken from vena cava; the concentration of endotoxin and motilin in plasma and the level of gastrin, IL-1β and IL-6 in serum were measured. Pathologic changes of pancreas, lung, intestine and liver were observed under optical microscope. Results Compared with NS group, pathologic demage of pancreas and pulmonary was less severe, and pancreatic haemorrhage was reduced. The concentation of endotoxin, gastrin, IL-1β, IL-6 were significantly reduced (P<0.01) and level of motilin in the blood of both portal vein and vena cava was increased. Conclusions Chinese Medicine Qingyi tang can reduce the production of proinflammatory mediators, ameliate the damage caused by endotoxin, and protect the pancreas and pulmonary from injuries in SAP.
