全软成形环在心脏瓣膜关闭不全成形术中的应用

Clinical application of totally flexible prosthetic ring for annuloplasty of mitral insufficiency

目的 :评价全软成形环在二尖瓣关闭不全瓣环成形术中的作用。方法 :对 2 8例二尖瓣关闭不全患者施行全软成形环瓣环成形术 ,手术前后检测多普勒超声。结果 :全组无手术死亡病例。术后二尖瓣口面积由术前的(1.6 6± 0 .6 5 )cm2 扩大至术后 (2 .6 5± 0 .39)cm2 (P <0 .0 5 ) ,二尖瓣跨瓣压力阶差由术前的 (2 .37± 0 .87)kPa降至术后的(0 .6 7± 0 .2 9)kPa(P <0 .0 5 )。多普勒超声和临床数据均显示全软成形环具有生理功能 ,符合二尖瓣环生理 ,且瓣膜关闭良好 ,患者临床症状明显改善。结论

Aim: To assess the effect of the totally flexible prosthetic ring in mitral and tricuspid annuloplasty. Methods: A total of 28 patients with mitral and tricuspid insufficiency underwent annuloplasty using totally flexible prosthetic ring. Color Doppler echo cardiography was performed in all patients before and after surgery. Results: No operative mortality occurred. The mean areas of mitral valve increased from (1.66±0.65)cm 2 to (2.65±0.39)cm 2(P<0. 05),the mean gradients of mitral valve declined from (2.37±0.87)kPa to (0.67±0.29)kPa (P<0. 05). All echocardiographic data and clinical data showed that the totally flexible prosthetic ring maintained physiological functions.The patients' cardiac functions were improved. Conclusion: The totally flexible prosthetic ring is safe and satisfactory for annuloplasty of mitral insufficiency.
