特殊部位异位妊娠的临床分析

Clinical analysis of special-site ectopic pregnancy

目的 探讨特殊部位异位妊娠的临床特点 ,误诊原因及治疗原则。方法 对 1996年 1月至2 0 0 3年 12月诊治的 2 5 6例特殊部位异位妊娠进行回顾性分析。结果 特殊部位异位妊娠发生率占同期异位妊娠的 3 36 % ,术前诊断率 4 8 4 % ,所有的患者均行手术。视术中情况进行不同的处理 ,所得标本均经病理切片证实 ,全部患者愈后良好。结论 特殊部位异位妊娠的临床表现不典型 ,容易漏诊误诊 ,一旦怀疑本病应尽早住院 ,及时明确诊断并手术 。

Objective To explore the clinical characteristics and reasons for misdiagnosis of special-site ectopic pregnancies.Methods The retrospective analysis was performed on 256 special-site ectopic pregnancies in our hospital from January 1996 to December 2003.Results The incidence of special-site ectopic pregnancies was 3.36%,and the preoperative diagnosis rate was 48.4%.All the patients received emergency exploratory laparotomy.Then according to finding at surgery,some patients received focal resection of the ectopic pregnancy,and if necessary,the patients underwent resection of the appendix and total hysterectomy.Conclusion The clinical features of special-site ectopic pregnancy are not typical,so it is easy for misdiagnosis .If there is a suspicion of this condition,surgical exploration should be preformed early to get better therapeutic effects.
