组织间激光凝固治疗恶性实体瘤

Therapy of malignant tumors through interstitial laser photocoagulation

摘要 目的评价激光消融对恶性实体瘤治疗的临床价值.方法采用半导体激光对122例恶性实体瘤病人的146个病灶进行激光消融治疗.使用功率2~10 W,能量为7200~14400J,消融时间1 200s.治疗后(1,2,3)个月予CT复查,评价肿瘤病灶坏死程度.结果1个月后CT复查示146个病灶均有不同程度的缩小,其中完全坏死的病灶为28个(19.1%),接近完全坏死(90.0%~99.0%)的病灶为42个(28.7%),部分坏死(50.0%~89.0%)的病灶为76个(52.2%).结论激光消融是治疗非手术切除的小恶性实体瘤的安全而有效的方法之一,但对直径大于3 cm的恶性实体瘤,应采用多点、多次的方法消融治疗. Objective:To evaluate the clinic value of the treatment to malignant tumors through laser ablation.Methods:There are122patients with146focuses were ablated by semicondutor laser.The power was2~10W.The dose was7200~14400J and the ablation time was1200seconds.CT scan was done after the treatment for1,2and3month.The necrotic degree of the tumor focuses were evaluated.Results:The146focuses dwindled with different degree after one month through the evaluation of CT scan.Among of them,there are28(19.1%)focuses were completely necrotic,42(28.7%)focuses were nearly completely necrotic(90%~99%)and76(52.2%)focuses were partly completely necrotic(50%~89%).Conclusions:The laser ablation can cure the small inoperable malignant tumors and the method was safe and effective.The laser ablation is made through more points and time after time when the diameter of the tumors exceed3cm.
