双水平气道正压通气治疗慢性阻塞性肺疾病Ⅱ型呼吸衰竭 被引量:4

摘要 目的观察双水平气道正压辅助通气治疗慢性阻塞性肺疾病急性加重期Ⅱ型呼吸衰竭的疗效。方法对19例慢性阻塞性肺疾病急性加重期Ⅱ型呼吸衰竭患者,在常规抗感染及综合治疗的基础上,给于Bi-PAP辅助通气,并观察治疗前后血气指标和症状体征的变化和疗效。结果治疗后pH值无明显变化,PaO2明显上升(P<0.001),而PaCO2有明显下降(P=0.049);19例患者中15例病情明显好转,顺利脱机。结论无创BiPAP辅助通气治疗性阻塞性肺疾病急性加重期Ⅱ型呼吸衰竭疗效确切。 Objective:To approach applied effect of Bi-level positive airway pressure(BiPAP)ventilation for COPD in acute typeⅡrespiratory failure.Methods:19cases of COPD with acute typeⅡrespiratory failure were treated with Bi-level positive airway pressure ventilation when routine medical treatment were performed,the blood gas and general applied value were evaluated before and after treatment.Results:Compared with before treatment,PaO 2 were markedly risen(P<0.001),and PaCO 2 were markedly descended(P=0.049)after treated with BiPAP,there were no significant change in pH.Among them,15patients were successfully weaned when improved.Con-clusions:Non-invasive BiPAP ventilation have better curative effect for COPD in acute typeⅡrespiratory failure.
