腹腔镜下骶前神经切断术与宫骶韧带切断术治疗子宫内膜异位症痛经的对比研究

Investigation of the effects of presacral neurectomy and uterosacral ligament amputation on dysmenorrhea of pelvic endometriosis by laparoscope.

目的 比较和评价骶前神经切断术与宫骶韧带切断术治疗盆腔子宫内膜异位症所引起痛经的效果。方法 2 0 0 0年 3月至 2 0 0 3年 1 0月对深圳市罗湖区人民医院 55例盆腔子宫内膜异位症所致痛经病人行骶前神经和宫骶韧带切断术 ,其中骶前神经切断术 2 1例 ,宫骶韧带切断术 3 4例 ,对两组的手术情况、手术效果、术后随访等进行对比研究。结果 两组病人术后观察 3～ 2 4个月经周期 ,中位随访时间为 1 0个月 ,其中骶前神经切断组有效率为 95% ,宫骶韧带切断组效率为 67%。结论 腹腔镜下骶前神经切断术治疗子宫内膜异位症所引起痛经的疗效明显高于宫骶韧带切断术。

Objective To Compare and assess the effectiveness of presacral neurectomy and uterosacral ligament amputation in women with dysmenorrhea caused by pelvic endometriosis.Methods We observed 55 patients with pelvic endometriosis in our hospital from March 2000 to October 2003,21 of which received presacral neurectomy and the others uterosacral ligament amputation.Operation condition,effectiveness of releasing pain,postoperative follow up were compared between the two groups.Results After 3 to 24 months' observation,in presacral neurectomy group,rate of effectiveness was 95%.While in uterosacral ligament amputation group,rate of effectiveness was 67%.Conclusion Presacral neurectomy is more efficient than uterosacral ligament amputation in patients with dysmenorrhea caused by pelvic endometriosis.
