慢性乙型肝炎患者肝组织炎性活动程度与α-干扰素疗效的关系

Relationship between Histology of Liver Tissue and Effect of Interferon-alpha Treatment in Patients with Chronic Hepatitis B

目的 :探讨慢性乙型肝炎 (ChronichepatitisB ,CHB)患者肝组织炎性活动程度与α 干扰素 (α IFN )疗效的关系。方法 :对 84例使用赛诺金 (αlb IFN )的CHB患者治疗前肝活检组织进行炎症分级及Ishak计分 ,并与α IFN治疗后及停药 1年后复发情况作一对比分析。结果 :随着肝组织炎症分级进展 ,α IFN的完全应答率明显上升( χ2 =11 63 ;P <0 0 1)。停药 1年后不同炎症分级患者的HBV感染复发率并无差异。 8例完全应答者α IFN治疗结束时 ,肝组织Ishak计分自 12 8± 3 0降至 8 1± 2 8(t =3 2 4;P <0 0 1)。结论 :CHB肝组织炎症分级与α IFN近期疗效密切相关。α

Objective:To evaluate the relationship between histology of liver tissue and effect of Interferon-alpha(α-IFN) treatment in patients with chronic hepatitis B(CHB). Methods:The liver biopsy specimens from 84 CHB patients was assessed using inflammatory activity grading and Ishak score and compared with the effect of Interferon-alpha treatment. Results:The Ishak score in the group with full response of α-IFN was significantly higher than that in the group without full response. The full response ratio was markedly increased along with the evolvement of inflammatory activity grade. The Ishak scores of 8 cases with full response decreased from 12.8±3.0 to 8.1±2.8 after 48 weeks treated(t=3.24;P<0.01).Conclusion:The inflammatory activity grade was closely correlated with the short term therapeutic effect of IFN. The histology of liver tissue can be distinctly improved in the patients with full response.
