摘要 目 的 探 讨 P 波变 异 性(Pvari )对 阵 发性 心 房 颤 动的 预 测 价 值。 方 法 测 量 阵 发 性心 房 颤动 组 37 例 和 对照 组 36 例 的 Pvari、P 波最 大 时间(Pm )和 P 波离 散度(Pd)。以 Pvari≥120m s2 为阳 性 标 ax准 预 测,并与 Pm ≥110m s 或 Pd ≥40m s为 阳性 标准 比 较,评 估 Pvari 对阵 发性 心 房颤 动的 预 测 价值 。 同 ax时 比 较患 者 恢 复窦 性 心 律 后第 2 天 与 第 5 天 Pvari、Pm 及 Pd 的 重复 性 。 结 果 与对 照 组 比 较, 患 者 ax组 Pvari 、Pm 和 Pd 增 大 ,差 异 有 非 常显 著 性 意 义(P<0.01)。 以 Pmax ≥110m s 、Pd≥40m s、Pvari  ≥120m s2 为 ax阳 性 标 准 ,预 测 心房 颤 动 的 敏感 性 分 别 为 86.5% 、81.1% 、75.7% ,特 异 性 分 别 为 77.8% 、86.1% 、88.9% ,阳 性 预 测 值分 别 为 80.0% 、85.3% 、87.9% ,阴 性 预 测值 分 别 84.8% 、81.6% 、78.0% 。 ? Objective Aim was to study prediction value of variance of P wave duration(Pvari) for paroxysmal atrial fibrillation( PAf). Methods Maximum of P wave duration (Pmax), P wave dispersion(Pd) and Pvari were measured for 37 patients with PAf and 36 healthy controls.At meantime,second day's and fifth day's electrocardiograms after patients restored sinus rhythm were recorded to compare reproducibility of Pmax , Pd, Pvari. Predictive value of Pmax Pd and Pvari were evaluated using Pmax≥110ms, Pd ≥40ms or Pvari≥120ms2 as criteria. Results Pmax Pd and Pvari were significantly higher in PAf group than in controls group ( P<0.01). Pmax ≥110ms ?Pd≥40ms?Pvari ≥120ms2 separated patients from controls with a sensitivity of 86.5%, 81.1% ,and 75.7% respectively, a specificity of 77.8%, 86.1%, 88.9% respectively; a post predictive value of 80.0%, 85.7% ,87.9% respectively, a negative predictive value of 84.8%, 81.6% and 78.0% respectively. Short-term reproducibility of Pmax ,Pd and Pvari were good( r=0.78,0.80, 0.83,P<0.01). Conclusion Pvari was increaseded significantly in patients PAf, that suggested Pvari could be one of electrocardiographic makers to predict PAf.
