胃球样异型增生细胞凋亡的组织芯片观察

摘要 目的 应用组织芯片技术探讨胃黏膜球样异型增生细胞凋亡的特点。方法 使用原位末端标记法和免疫组织化学染色 ,检测 10 7例胃球样异型增生和 41例正常胃黏膜、12 4例胃癌的细胞凋亡和调控基因bcl 2、bax蛋白的表达情况。结果 轻度球样异型增生bcl 2 /bax阳性表达的比值与正常胃黏膜及胃癌比较 ,均有非常显著性差异 (t =2 .12 7,P <0 .0 5 ;t =2 3 .2 71,P <0 .0 1) ;重度球样异型增生者显著高于正常胃黏膜及轻度球样异型增生 (t =7.2 5 8,P <0 .0 1;t =2 .65 9,P <0 .0 5 ) ,而与胃癌比较无显著性差异 (t =1.5 66,P >0 .0 5 )。轻度和重度球样异型增生的细胞凋亡指数与正常胃黏膜比较有非常显著性差异 (t =7.5 5 2 ,t =19 .81,P <0 .0 1) ,与胃癌比较无显著性差异 (t =1.85 3 ,t =0 .694,P >0 .0 5 )。从正常胃黏膜、轻度球样异型增生、重度球样异型增生到胃癌 ,细胞凋亡指数以重度球样异型增生为高峰 ,呈峰型变化。结论 重度球样异型增生细胞的bcl 2表达、bcl 2 /bax比值和细胞凋亡指数均增高 。 Objective To exploe the significance of apoptosis in gastric glaboid dysplasia.Methods The number of the apoptpsis cell and the positive expression of bcl-2,bax protein were detected by TUNEL and immunohistochemical methods with tissue microarray in 107 cases of gastric glaboid dysplasia,41 cases of normal gastric mucosa and 124 cases of gastric carcinoma.Results The expression of bcl-2/bax was significantly different between the low-grade glaboid dysplasia and the normal gastric mucosa(t=2.127,P<0.05),or between the low-grade glaboid dysplasia and gastric carcinoma(t=23.271,P<0.01).The expression of Bcl-2/bax was markedly higher in high-grade glaboid dysplasia than in normal or low-grade glaboid gysplasia(t=7.258,P<0.01;t= 2.659,P<0.05),but was not significantly different from that in gastric carcinoma(t=1.566,P>0.05).The apoptotic index was significantly different between glaboid dysplasia and normal gastric mucosa(t=7.552,t=19.81,P<0.01),but showing no significantly different between glaboid dysplasia and carcinoma(t=1.853,t=0.694,P>0.05).Conclusion The expression of bcl-2,bcl-2/bax and the apoptotic index reached a peak value in the high-grade gastric glaboid dysplasia,which may be closely related to the carcinogenesis of gastric carcinoma.
