氟喹诺酮类抗生素的心脏毒性 被引量:18

摘要 虽然氟喹诺酮类抗生素的心脏不良反应罕见 ,但因后果严重 ,因此倍受关注。近年来对氟喹诺酮类药物的心脏毒性进行了深入研究 ,证实它们能阻断心室肌细胞膜上快速激活的延迟整流钾电流 (Ikr) ,使心肌复极时间延长 ,心电图表现为QT间期及QTc间期延长 ,在此基础上诱发致命性恶性室性心律失常 ,如多形性室性心动过速、尖端扭转型室性心动过速 (TdP)及心室颤动。这种毒性反应与氟喹诺酮类的结构密切相关 ,并呈剂量依赖性。本文介绍氟喹诺酮类药物心脏毒性的产生机理、发生率、诱发因素等 。
