合并神经系统损害的主动脉夹层30例临床分析

A Clinical Analysis of 30 Cases of Aortic Dissection with Neurological System Lesion

目的 探讨主动脉夹层 (aorticdissection ,AD)合并神经系统损害的病因、发病机制、临床特征及诊断要点 ,以提高对本病的认识。方法 回顾性总结分析收治的符合AD临床诊断的 30例合并神经系统损害患者的临床资料 ,其中神经系统损害排除了既往史中其他神经系统疾病。结果 本组 30例患者中以急性神经系统损害为首发症状者 9例 ,单纯神经系统损害者 2例 ,病程中伴有神经系统损害者 2 8例。结论 对于合并有胸背痛、胸闷、血压不对称、脉搏短绌等症、征的神经系统损害的患者要高度怀疑AD的可能 。

Objective To probe into the causes, pathogenesis, clinical manifestations and diagnosis outline of aortic dissection with neurological system lesion for further understanding of it. Methods To sum up and analyze 30 clinical cases which accord with the diagnosis standard of aortic dissection and incorporate with neurological system lesions, which were not caused by other neurological system disease in their anamnesis. Results The study indicated that the initial symptoms of 9 cases were neurological system manifestations, 2 cases only had simple neurological system lesions and 28 cases incorporated with neurological system lesions in their courses of disease. Conclusions The probability of AD should be highly suspected in the patients of neurological system lesions with chest pain, unsymmetrical blood pressure and insufficient pulse. So we can debase the leak rate and error rate.
