摘要
目的 探索癫痫患者的智力障碍特点和智力障碍与发作频率及痛程的关系。方法 选取驻马店市精神病医院司法医学鉴定的41例癫痫患者,采用中国修订韦氏成人智力量表(WAIS-RC)进行智商测验,并和35例正常人进行比较。结果两组言语智商(VIQ)、操作智商(PIQ)和总智商(FIQ)差异均有显著性(P<0.001);41例癫痫患者的智力障碍发生率高达41.5%(17/41),两组智力等级分布和智力障碍发生率均有显著差异(P<0.005);各分测验量表均分与正常对照组相比均有显著兰异(P<0.001),其中图片排列(5.6±3.6)、木块图(5.9±2.9)、相似性(6.5±3.4)和填图(6.9±2.7)均超过了一个标准兰。结论 癫痫发作频繁程度和病程长短对智力均有明显影响;癫痫所致智力障碍的特点是在全面智力受损的同时,其逻辑联想、部分与整体关系的观念、思维灵活性、抽象概括能力、空间关系能力、视觉分析和视觉辨认能力等智力因素受损可能更明显。
Objective To explore the characteristic of intellectual disorders of patients with epilepsy and the relationship between intellectual disorders of patients with epilepsy and teh relationship between intellectual disorders and attacking frequency or course. Methods 41 patients with epilepsy conducted by medicolegal forensic assessment in our hospital wee selected as the subjects. WAIS-RS was used to test the Intelligence Quotient (IQ), And 35 normal peoples were selected as control group. Results There are significant differences in VIQ,PIQ and FIQ between teh two groups(P<0. 001). The occurrence rate of intellectual disorders of patients with epilepsy has reached 41. 5% (17/41). There are significant differences in intellectual degree distribution and the occurrences of intellectual disorders between the two groups(P<0. 005). There were significant differences between the nominal control group and average score of each rating scale(P<0. 001). Picture Arrangement(5. 6±3. 6), Block Design(5. 9±2. 9) ,Similarities(6. 5±3. 4) and Picture Completion(6. 9±2. 7) have surpassed one standard deviation a mong them. Conclusion The attacking frequency and the course have influenced intelligence obviously ;The characteristic of intellectual disorders due to epilepsy is that the overall intelligence is impaired , while logical thinking ,conception on the relationship between part and whole, thinking flexibility ,abstract summarizing ability, space relationship ability ,visual analysis and visual distinguishable ability and so on may be impaired more obviously.
