摘要 【目的】探讨碱性成纤维细胞生长因子(bFGF)在实验性自体筋膜硬脑膜重建中的应用价值。【方法】取SD大鼠35只,分成5组。其中A组3只,直接将背部筋膜覆盖在硬膜外,B组5只,采用自体筋膜硬脑膜重建模型,通过免疫组织化学方法观察这两组大鼠自体筋膜硬脑膜移植的愈合过程及细胞因子bFGF在其中的表达;C组、D组、E组各9只,分别进行自体筋膜硬脑膜重建,D组加用外源性bFGF,E组仅用明胶海绵,通过脑脊液漏研究、免疫组织化学方法研究外源性bFGF对自体筋膜重建硬脑膜愈合过程的影响并用RT-PCR方法研究内源性bFGFmRNA的表达情况。【结果】大鼠自体筋膜移植硬脑膜部位bFGF表达明显;C、D、E组大鼠重建硬脑膜抵抗脑脊液漏压力值(mmH2O)分别为311±75,497±153,338±88,Ⅰ型胶原纤维表达值分别为4.9±0.8,10.9±1.6,5.3±0.9,D组均好于对照组(P<0.05);内源性bFGFmRNA的表达没有明显变化。【结论】在自体筋膜移植硬脑膜的愈合过程中bFGF可能起了重要的作用;外源性bFGF处理的大鼠其移植筋膜愈合要好于对照组。 ObjectiveTo study the effect of basic fibroblast growth factor (bFGF) on the experimental dural reconstruction achieved by a free fascial graft. MethodsA total of 35 SD rats were divided into 5 groups. In Group A (n=3), a piece of fascia was put outside the intact dura mater. The dural reconstruction models were created in Group B (n=5). The healing process and the effect of bFGF were studied by immunohistochemical method. In Group C, D, and E, (each n=9) dural reconstruction models were made and exogenous bFGF was administered in group D but only gelfoam was given in group E. The effect of exogenous bFGF was studied by cerebrospinal fluid (CSF) leak experiment and immunohistochemical method. The expression of endogenous bFGF was studied by reverse transcription polymerase chain reaction (RT-PCR).ResultsThe reconstructed dura was immunohistochemically stained with antibodies against bFGF. The pressure against CSF leak in group C, D, and E were 311±75, 497±153, and 338±88, respectively. The expression values of type-1 collagen fiber were 4.9±0.8,10.9±1.6,and 5.3±0.9, respectively. There was statistical difference between group D and the other two groups (P< 0.05). There was no difference among the three groups in the expression of bFGF mRNA. ConclusionsbFGF may play an important role in the healing process of free fascial graft after reconstruction and exogenous bFGF could promote the process.
