比格犬硬膜外腔造影误入蛛网膜下腔致脑栓塞的诊治与临床借鉴

摘要 目的 探讨造影剂误入动物蛛网膜下腔后导致脑栓塞形成的抢救和预防。方法 参照人类脑水肿治疗方案和剂量 ,采用连续缓慢注入 2 0 %甘露醇和 10 %葡萄糖酸钙于比格犬前肢皮下静脉内 ,直至该犬大量排尿及身体抽搐基本停止的抢救措施。同时改用小剂量、低粘度碘造影剂 ,并抬高动物头部的方法来预防脑栓塞形成。结果 采用抗脑水肿治疗效果明显而通过肌内注射冬眠灵和安定的抗惊厥处理是无效的。造影剂所致动物脑栓塞的形成能通过上述方法有效地预防。结论 本文所述脑栓塞形成的预防和抢救是有效和成功的 ,它应同样适用于临床同类病人的救治。 Objective To investigate the methods of salvage and prevention of animal's brain infarction induced by cardiografin into subarachnoid space. Methods According to the procedure of treatment of human hydrocephalus, 20% mannitol and 10% calcium gluconate were slowly injected in the subcutanous vein of forelimb of animals repeatly until the animal started urenation a lot and stoped body convulsion. Besides, low dose and less stiff iodine cardiografin was used with the animal's head raised in order to prevent the brain infarction. Result The anti-convulsion treatment via injection of wintermin and diazepam in the animal muscle was useless, whereas the effects were obvious by adapting anti-hydrocephalus treatment. The formation of the brain infarction could be effectively prevented by the above method. Conclusion The prevention and salvage treatment for animal’ brain infarction induced by cardiografin were effective and successful. This result would be adapted in clinic.
